Role & seniority: Quality Assurance Coordinator (Full-Time)
Stack/tools: Electronic Health Record (EHR) quality monitoring; compliance with healthcare regulations; performance-improvement data collection and analysis; knowledge of Title 9, Title 22, OSHA, HCAI, HIPAA, CARF
Monitor and audit standards of person-served care and the overall quality of the EHR; ensure QA standards are current and compliant with licensing/accrediting policies
Lead/participate in quality improvement activities, infection control, medical records, and staff development; collaborate with the facility QA committee
Ensure adherence to regulatory requirements and support implementation of QA plans
Degree in a health-related field and/or two years of health care education
Minimum one year in medical records and/or quality improvement; experience with healthcare/regulatory agencies
Ability to collect, analyze, and apply performance-improvement data; strong regulatory knowledge (Title 9/22, OSHA, HCAI, HIPAA, CARF)
Nice-to-haves: Knowledge of or experience with behavioral health facilities; strong communication and collaboration skills

Title: Quality Assurance Coordinator
Job Duties: The Quality Assurance (QA) Coordinator plays an integral part in the overall functioning of the facility by monitoring and auditing the identified standards of person-served care as well as the quality of the entire electronic health record. The QA Coordinator will ensure quality improvement standards are up to date and in compliance with the facility's level of care. This position encompasses aspects of staff development, infection control, medical records, and quality improvement to effectively collaborate with the QA committee and program at the facility level. The QA Coordinator is responsible for monitoring overall compliance with these aspects as they relate to current licensing and accrediting policies and regulations.

About the Campus: The Crestwood Center San Jose Psychiatric Health Facility (PHF) on our San Jose campus is in close proximity to the mental health support services in Santa Clara County. The San Jose PHF provides a spacious environment, with two living rooms, a welcoming room, visiting space, serenity room, dining room and a beautiful courtyard. The program is dedicated to restoring a sense of hope, self-empowerment and recovery in each of our clients. The San Jose PHF is designed for clients who require stabilization at a time of acute psychiatric crisis. We provide medication management, WRAP, DBT support, welcoming rituals, behavioral interventions, psychotherapy, psychosocial education and a thorough transition plan to enable a successful return to the community. Mental Health Rehabilitation Centers (MHRCs) provide psychosocial rehabilitation programs in secure, residential settings with a focus on brief lengths of stay. The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.
